Various Report Narratives
BURGLARY (AUTO)
The victim (resident at location, employee at location) stated that between the indicated times, person(s) unknown entered his locked (vehicle description, license number ) and stole the above described items.
Entry was gained via (side door, window, etc.) by (breaking window, prying window, etc.), reaching in and opening the door.
I/We saw (broken window, pry marks).
Report memo issued to the victim.
BURGLARY (STRUCTURE)
The victim (resident, employee) at location stated that between the indicated times, person (s) unknown entered the location and stole the above described property.
Entry into the location was gained via (indicate point of entry).
Exit was apparently made via( indicate point of exit).
Items taken were (item number and the location of item).
I / We saw (pry marks, broken window, etc.)
Print Deputy was/was not requested due to (lack of etc.)
Report memo was given to the victim.
EMBEZZLEMENT
The informant (manager, owner, employee) stated that between the above indicated times, the suspect took (list item ) that was entrusted to him/her.
The informant also said that the suspect (indicate the last day the suspect worked, was seen, or spoken to; and include any information that shows how the informant found out the suspect was gone or how the suspect committed the crime).
List any witnesses, or others who last saw the suspect.
Obtain an inventory of the embezzled items , if possible.
ARMED ROBBERY
The victim (resident, employee) at location stated that at the indicated time, the suspect(s) entered the location, walked/ran over to the counter, where he was standing and pointed a (weapon used) at him. The suspect ordered/demanded the victim to give him all the money in the (register, drawer, etc.)
Fearing for his life and safety, the victim complied.
The suspect made the victim ( lie down, stand in a corner, etc. --- indicate the position in which the victim was left) at which time the suspect fled the location in (direction or vehicle used, if any --- indicate unknown direction).
A crime broadcast was made (indicate by whom and how).
We saw the following:

211 PURSE SNATCH
The victim stated that at the above indicated time, while walking (direction) on (street), the suspect approached from the (rear, side, front) and grabbed her purse.
As the victim held on to her purse, the suspect knocked her to the ground, pulled the purse from her grasp and fled (indicate direction of travel, identify the street, and means of transportation).
A crime broadcast was made (indicate by whom and how).
A search of the area (by whom and what was the result).
The victim can identify suspect.
A report memo was given to the victim.
GRAND THEFT or PETTY THEFT
The victim (resident, employee, etc.) stated that between the indicated times, person (s) unknown took the above described property from (where items were taken from).
I/We saw (Indicate any signs or personal observations that substantiate the theft).
A report memo was given to the victim.
FIRE REPORT
Upon our arrival at the location, we saw Engine Company ______ under the direction of Captain __________, extinguishing the fire.
After the flames were extinguished, we saw that the (NW, SE etc.) portion of the interior was (gutted/destroyed/damaged) by fire. We also saw that the (adjacent wall, ceiling, structure, etc.) was (charred/blistered,/totally destroyed). The interior of the location was damaged by smoke.
Captain ___________ estimated the structural damage at $_________. He requested a (indicate what type of investigation he requested). Captain _______ feels the fire was started by (indicate cause of the fire and list the reasons provided by the fire Captain).
If the victims are at the scene, get the insurance company's name and find out who will secure the location.

GRAND THEFT AUTO (ROLLING)
While driving (indicate direction) on (identify street) in our marked patrol unit, we saw the above described vehicle (indicate what the vehicle was doing and provide the appropriate vehicle code violations if applicable).
A check of the vehicle's license plate via our M.D.T. revealed that it was a (identify law enforcement agency) stolen, reported (date stolen) under (file number).
A felony traffic stop was conducted with the assistance of (list assisting units).
The driver and sole occupant (if applicable) of the vehicle was (indicate what happened).
Example: The driver and sole occupant of the vehicle was ordered out of the car and detained without incident.
Upon further investigation, we determined the suspect was not the R/O of the vehicle and that the vehicle was in fact stolen.
CONCLUSION FOR AN ADULT ARREST
Due to the above, the suspect was arrested, transported to and booked at (station) with the approval of (person's title and name ).
CONCLUSION FOR A JUVENILE ARREST
Due to the above the subject was arrested. Per Shad- 477, I/WE advised of him/her of his/her constitutional rights.
He was transported to and booked at (station) with the approval of Lt. ___________._ __________ Detective________ notified.
ROUTINE STOLEN
The Victim stated that between the indicated times, person (s) unknown stole the above described vehicle from (location car was stolen from). I saw (indicate what evidence you saw or didn't see). R/O information was obtained from (victim's memory, pink slip or registration in victim's possession) and confirmed via C.L.E.T.S. . No other wants S.V.S. Vehicle was entered into S.V.S. as stolen by sec'y at ____ hours.
STORED\IMPOUNDED VEHICLE
Driver (name) was cited for (list violations) cite #________. Vehicle was towed pursuant to ( provide vehicle code violation). R/O info obtained and verified via C.L.E.T.S., no wants S.V.S. vehicle was entered into S.V.S. as stored by sec'y __________at ______ hours.
DRIVER ARRESTED
Driver (give name) arrested for (give P.C. section) and booked at (identify station). R/O/ information obtained from registration in vehicle (or indicate how this info was obtained) and confirmed via C.L.E.T.S. . No other wants S.V.S. The vehicle was entered into S.V.S. as (stored, impounded, evidence). Refer to Shad-49 under above file number for details.
IMPOUNDED USED IN A CRIME
Vehicle was impounded and held as evidence re: used in a (provide the penal code). R/O info obtained and verified via C.L.E.T.S.. No other wants S.V.S. The vehicle was entered into S.V.S. by (station) sec'y ________ at ______hours. Refer to report under the same file number for further information.
IMPOUNDED VEHICLE EXPIRED REGISTRATION
Vehicle was impounded pursuant to 22651(o) V.C., registration is expired in excess of one year. R/O info obtained and verified via C.L.E.T.S. . No other wants S.V.S. Vehicle entered into S.V.S. by (station) sec'y _______ at ______ hours.
RECOVERY REPORT
